Description

Specialized Offloading for the Diabetic Foot

The OPED VACOcast Diabetic Offloading Walking Boot combines a rigid knee-high shell with VACO12 vacuum technology to provide adaptable, total-contact pressure distribution. It is intended for clinician-directed management of diabetic foot ulcers, Charcot foot and other complex diabetic foot conditions.

More Than a Standard Walking Boot

The diabetic model has a higher-volume vacuum insert, low-friction velour liner, additional foam footbed and protective toe cover. These features distinguish it from the standard VACOcast fracture boot and help address the pressure-management and hygiene requirements of diabetic foot care.

What Is the VACOcast Diabetic Boot Used For?

The VACOcast Diabetic is a specialized knee-high offloading orthosis. Its purpose is to reduce and redistribute plantar pressure while stabilizing and protecting the foot and ankle during treatment.

OPED lists the following indications:

  • Diabetic Foot Syndrome
  • Foot ulcerations requiring offloading
  • Post-operative care after toe or forefoot amputation
  • Diabetic neuro-osteoarthropathy
  • Diabetic neuropathic osteoarthropathy, also known as DNOAP
  • Charcot foot

Professional Assessment and Fitting Required

Diabetic wounds, reduced sensation, infection, circulation problems and Charcot changes require ongoing medical supervision. This orthosis should be selected, fitted and monitored by a physician, podiatrist, wound-care specialist, orthotist or other qualified healthcare professional.

How VACO12 Vacuum Offloading Works

The diabetic vacuum insert contains thousands of small beads. With the valve open, the beads can move so the insert conforms around the individual anatomy. Air is then removed with the supplied fitting pump, fixing the beads into a supportive shape.

This creates a broad contact surface around the foot and lower leg. Instead of concentrating force beneath a smaller area, pressure can be redistributed across a larger supporting surface.

VACO12 FeatureClinical Purpose
Movable bead fillingConforms around individual foot and lower-leg anatomy before the vacuum is created.
Vacuum-stabilized insertHolds the moulded shape to provide stable, total-contact support.
Reformable constructionAllows the fit to be adjusted as swelling, dressings or clinical requirements change.
Broad supporting surfaceHelps redistribute pressure away from vulnerable areas of the foot.
Knee-high rigid shellProvides stabilization and limits unwanted foot and ankle movement.

What Makes This Model Diabetic-Specific?

ComponentVACOcast Diabetic Design
VACO12 vacuum insertContains additional bead volume for comprehensive pressure distribution and offloading.
Velour linerProvides a soft, low-friction interface around the vacuum insert and lower leg.
Foam footbedAdds cushioning and can be adapted by the treating professional for localized pressure relief.
Toe coverHelps protect the toes and reduce entry of dirt or debris.
Rocker walking soleSupports rollover during approved weight bearing and can be removed for cleaning.
Optional strap lockAllows a healthcare professional to convert the boot into a non-removable offloading device when clinically appropriate.

Do Not Substitute a Standard Fracture Boot

The regular VACOcast does not contain the same diabetic-specific insert, velour interface, foam footbed and pressure-management features. A standard walking boot should not be substituted when a diabetic offloading orthosis has been prescribed.

Removable or Clinician-Locked Offloading

The VACOcast Diabetic may be used as a removable walker or converted into a non-removable device using OPED’s optional strap-lock system. This decision belongs to the treating professional and depends on the wound, circulation, infection status, treatment plan and ability to follow the prescribed wearing schedule.

The optional strap lock is not included with the standard boot. Patients must not create their own locking system or modify the straps.

Measurements Take Priority Over Shoe Size

Shoe size is only an initial reference. Calf circumference, ankle circumference, foot width, dressings, swelling and deformity can all affect selection. If measurements overlap two sizes or fall near a limit, contact The Physio Store or the treating professional before ordering.

General Application Process

Initial application should be completed or supervised by a healthcare professional familiar with diabetic foot offloading.

  1. Inspect and dress the foot according to the wound-care plan.
  2. Open the vacuum valve so the bead filling can move freely.
  3. Distribute the beads evenly through the insert.
  4. Position the foot fully back in the liner and orthosis.
  5. Secure the shell and straps in the correct sequence.
  6. Mould the vacuum insert around the foot and lower leg.
  7. Remove air using the supplied fitting pump.
  8. Close the valve and attach the rocker walking sole.
  9. Confirm that the heel is seated and no localized pressure point is present.
  10. Reassess the skin, circulation, wound area and walking pattern.

The vacuum shape may require reassessment as swelling changes or dressings are replaced. Do not independently add padding, cut the footbed or change the vacuum distribution unless instructed by the treating professional.

Important Use and Skin-Monitoring Information

  • Follow the prescribed wearing and weight-bearing schedule.
  • Do not remove the boot simply because the foot feels comfortable.
  • Never place the liner or shell directly against an uncovered wound.
  • Keep all wound dressings in place as directed by the wound-care professional.
  • Inspect the skin and wound at the frequency recommended by the treating team.
  • Continue using prescribed crutches, a walker or another mobility aid.
  • Do not run, jump, walk quickly or participate in sports while wearing the boot.
  • Do not drive unless a healthcare professional has confirmed that it is safe.
  • Have the orthosis inspected if it is damaged or involved in a fall.

Report New Symptoms Promptly

Stop weight bearing and contact the treating healthcare professional if there is new redness, discolouration, drainage, odour, swelling, temperature change, increasing pain, numbness, altered sensation, skin breakdown, wound deterioration or a persistent pressure point. Seek urgent medical care for signs of spreading infection or impaired circulation.

Cleaning and Ongoing Care

  • Remove the liner only according to the healthcare professional’s instructions.
  • Close zippers and hook-and-loop fasteners before washing.
  • Follow the supplied manual for the permitted washing temperature and cycle.
  • Allow textile components to air-dry completely.
  • Do not tumble-dry or place components directly on a heater.
  • Wipe the rigid shell and walking sole with a suitable damp cloth.
  • Do not sterilize the orthosis.
  • Inspect the liner, vacuum insert, valve, straps and sole regularly for damage.
  • Use only OPED-approved diabetic replacement components.

The bead filling may gradually shift during use. The treating professional may instruct the patient or caregiver to open the valve, redistribute the beads and recreate the vacuum periodically.

Is the VACOcast Diabetic a total contact cast?

It is not a traditional plaster or fibreglass total contact cast. It is a vacuum-moulded knee-high offloading orthosis designed to provide cast-like stability and total-contact pressure distribution while allowing wound access when used as a removable device.

Can the VACOcast Diabetic be made non-removable?

Yes. A healthcare professional can use OPED’s optional strap-lock accessory to make the orthosis non-removable. The lock is not included with the standard boot and should only be applied as part of a supervised treatment plan.

Can I remove the boot to sleep or rest?

Only remove the boot according to instructions from the treating healthcare professional. The prescribed wearing schedule depends on the wound, Charcot activity, surgical procedure and overall treatment plan.

Can I walk immediately in the VACOcast Diabetic?

Only if weight bearing has been approved. Some patients may require non-weight-bearing or partial weight-bearing use before progressing. Continue using prescribed crutches, a walker or another mobility aid.

How do I choose the correct VACOcast Diabetic size?

Use calf circumference, ankle circumference, foot width, dressings and swelling in addition to shoe size. Shoe size is only an orientation guide. A healthcare professional should confirm the size whenever possible.

What should I do if my measurements fit two sizes?

Contact The Physio Store or the treating professional before ordering. OPED’s regional instructions differ on whether to size up or down at a boundary, and deformity, dressings, swelling and calf size may change the appropriate choice.

Is the VACOcast Diabetic universal for the left or right foot?

Yes. Each size can be used on either the left or right foot.

What comes with the VACOcast Diabetic Walking Boot?

The standard Canadian package sold by The Physio Store includes one boot system, one diabetic VACO12 vacuum insert, one diabetic velour liner, one foam footbed, one toe cover, one removable rocker sole and one fitting pump.

Is the strap lock included?

No. The OPED strap lock is an optional clinical accessory. It should only be selected and installed by a healthcare professional who wants to convert the boot into a non-removable offloading device.

Can I use a regular VACOcast liner in the diabetic boot?

No. Use the diabetic-specific velour liner with the diabetic vacuum insert. The standard terry-cloth VACOcast liner is not an equivalent replacement for the diabetic interface.

Can replacement diabetic liners and vacuum inserts be purchased?

Yes. The Physio Store sells diabetic-specific replacement velour liners and VACO12 vacuum inserts. Match the replacement component to the Small, Medium or Large boot size.

Can I wear the boot directly over an open ulcer?

No. The boot and liner must not be placed directly against an uncovered wound. The wound should be assessed and dressed according to the treating professional’s instructions before the orthosis is applied.

How often should my skin and wound be checked?

Follow the inspection schedule provided by the wound-care team. Patients with reduced sensation may not feel excessive pressure, rubbing, heat or moisture, so regular visual assessment is especially important.

What symptoms require medical attention?

Contact the treating professional promptly for new redness, discolouration, swelling, drainage, odour, temperature change, numbness, altered sensation, increasing pain, skin breakdown, wound deterioration or a persistent pressure area. Seek urgent care for signs of spreading infection or impaired circulation.

Can I adjust the vacuum insert myself?

Only if the treating professional has taught you how and has authorized home adjustment. Incorrect bead distribution can create unwanted pressure areas.

Can the liner be washed?

Yes. Close the fasteners before washing and follow the supplied user manual. Allow the liner to air-dry completely and do not tumble-dry it or place it directly on a heater.

Can I drive while wearing the boot?

Do not drive unless a healthcare professional has confirmed that it is safe and you can operate the vehicle normally. A right-sided walking boot is particularly likely to interfere with safe pedal operation.

Why might I need an EVENup shoe lift?

A knee-high walking boot raises the treated side and can create a temporary leg-length difference. An EVENup worn over the opposite shoe may improve walking symmetry and comfort when approved by the treating professional.

Do I need a prescription to purchase the VACOcast Diabetic?

A prescription is not normally required simply to complete the purchase. Professional assessment is strongly recommended, and a prescription may be required for insurance reimbursement or tax-exempt processing.

Is the VACOcast Diabetic tax-exempt in Canada?

A non-custom orthopaedic device may qualify for zero-rated GST/HST processing when supplied on the written order of a medical practitioner for the named consumer. Upload the prescription through the product page so The Physio Store can review it before processing the order.

Can I return the boot after opening the package?

No. Because this is a patient-contact medical orthosis, returns are limited to new, unopened products requested within five days of delivery. Security seals and packaging must remain intact.
