Fixed 90° Stability with a Vacuum-Contoured Fit
The OPED VACOcast Fracture Walking Boot is a tall, cast-stable foot and ankle orthosis with a four-strap shell, patented vacuum-bead insert and detachable rocker sole. It is designed for clinician-directed immobilization following selected fractures, severe sprains and foot or ankle procedures.
VACOcast or VACOped XELGO?
Choose VACOcast when the prescribed treatment plan calls for rigid support with the ankle held at a fixed 90° position. If an Achilles tendon protocol requires plantar-flexion positioning, wedges or adjustable range of motion, consider the OPED VACOped XELGO Achilles Walking Boot and confirm the required configuration with the treating healthcare professional.
What Makes the OPED VACOcast Different?
Many conventional walking boots use inflatable air cells that press inward around the leg. VACOcast uses OPED’s vacuum technology instead. Its internal cushion contains thousands of small beads that can move while the cushion is open and flexible.
Once the foot and lower leg are correctly positioned, air is removed using the supplied hand pump. The beads settle around the patient’s anatomy and become firm, creating a supportive custom contour around the limb.
The vacuum insert can be released and readjusted as swelling, muscle size or fit changes during recovery. Any substantial refitting should follow the instructions supplied with the boot or guidance from the treating professional.
VACOcast Features and Patient Benefits
| Feature | How It Helps |
|---|---|
| Fixed 90° ankle position | Provides rigid immobilization when a neutral ankle position is prescribed. |
| Vacuum-bead insert | Conforms around the individual shape of the foot, ankle and lower leg. |
| Adjustable vacuum fit | Can be remoulded to accommodate changes in swelling or lower-leg size. |
| Tall rigid shell | Helps protect and stabilize the foot, ankle and lower leg. |
| Four-strap closure | Helps secure the limb within the front and rear shell components. |
| Detachable rocker sole | Supports a smoother rollover during approved weight bearing and can be removed when not required. |
| Removable cloth liner | Provides a soft, absorbent interface and can be washed for improved hygiene. |
| Universal design | The same model can be worn on the left or right foot. |
Use According to Your Treatment Plan
VACOcast does not automatically mean that full weight bearing is permitted. The amount of weight you may place through the injured leg, how long the boot should be worn and whether it can be removed must be determined by your physician, surgeon, physiotherapist or other treating healthcare professional.
What Conditions May VACOcast Be Prescribed For?
A healthcare professional may prescribe the OPED VACOcast for conditions including:
- Ankle fractures
- Tarsal and metatarsal fractures
- Calcaneal or heel fractures
- Selected foot fractures
- Selected lower-leg fractures
- Severe ankle sprains
- Complex ligament injuries requiring immobilization
- Recovery following ankle joint replacement
- Upper or lower ankle arthrodesis
- Selected post-operative foot and ankle treatment
- Splinting or immobilization of the foot and lower leg
VACOcast is a medical orthosis and should not be used to self-diagnose an injury. Fractures, severe sprains, suspected tendon injuries and post-operative conditions require professional assessment.
VACOcast vs. VACOped XELGO
| Feature | VACOcast | VACOped XELGO |
|---|---|---|
| Primary positioning | Fixed at 90° | Fixed position or adjustable range of motion |
| Common treatment focus | Foot and ankle fractures, severe sprains and immobilization | Achilles rupture protocols plus selected fractures and ankle injuries |

OPED VACOcast Size Guide
| Size | Men’s US Shoe Size | Women’s US Shoe Size | Foot-Length Settings | Foot Width | Ankle Circumference | Calf Circumference |
|---|---|---|---|---|---|---|
| Small | 3.5–7.5 | 4.5–8.5 | 9.3 / 9.7 / 10.1 in | 4.1 in | 5.5–11 in | 9–15 in |
| Medium | 7.5–11.5 | 8.5–12.5 | 10.4 / 10.8 / 11.2 in | 4.7 in | 8–16 in | 13.5–20 in |
| Large | 11.5–15 | 12.5–16 | 11.3 / 11.7 / 12.2 in | 5.3 in | 12–18 in | 15–23 in |
Between Two VACOcast Sizes?
OPED generally recommends considering the smaller size when a shoe size falls directly on a boundary. However, foot width, ankle circumference and calf circumference must also be considered.
Contact The Physio Store before ordering if your measurements are close to a boundary or if you have an unusually narrow or large calf. Liner extensions and longer straps may be required for some users.
General VACOcast Application
- Follow the positioning and weight-bearing instructions provided by the treating healthcare professional.
- Open the straps and front shell, then release the vacuum so the bead insert becomes flexible.
- Smooth the beads through the foot and ankle areas before placing the foot fully into the liner.
- Position the heel securely at the back of the boot and place the liner inside the rear shell.
- Apply the front shell and fasten the four straps so they are secure but not excessively tight.
- Use the supplied pump as directed to remove air and form the vacuum insert around the limb.
- Recheck heel position, circulation, skin condition and overall comfort before walking.
This overview does not replace the OPED user manual or professional fitting instructions.
Cleaning and Daily Care
- Remove the vacuum insert from the cloth liner before washing the liner.
- Hand wash or machine wash the cloth liner using cold water and mild detergent.
- Air-dry the liner completely.
- Do not place the liner in a tumble dryer because heat may cause shrinkage.
- Wipe the plastic shell and sole using a damp cloth and mild soapy water.
- Allow all components to dry completely before reassembly.
- Check the skin regularly for redness, pressure areas, irritation or breakdown.
Shop the OPED replacement cloth liner for VACOcast.
Can You Swim in the VACOcast?
OPED states that the VACOcast shell and vacuum components can be used in water, but the cloth liner retains water. Swimming must first be approved by the treating surgeon or healthcare professional, particularly when an incision, wound or damaged skin is present.
A dry replacement liner is strongly recommended. After leaving the water, sit in a safe location, dry the foot and skin, towel-dry the shell and vacuum insert, and transfer the insert into the dry liner. The wet liner must be allowed to air-dry completely.
Permission to use VACOcast in a pool does not automatically mean that standing in a shower, entering open water or walking on wet surfaces is safe.
Walking More Evenly with a Medical Boot
A tall walking boot raises the injured side and can create a temporary leg-length difference. This may alter walking mechanics and place additional strain on the lower back, hips or knees.
The OPED EVENup Shoe Lift fits over the regular shoe on the non-injured side and may help create a more level walking position when approved by the treating professional.
Select the EVENup according to the regular shoe worn opposite the VACOcast—not according to the size of the walking boot.

Frequently Asked Questions
Have questions about sizing, vacuum fitting, weight bearing, cleaning or choosing between VACOcast and VACOped?
What is the OPED VACOcast Walking Boot?
The OPED VACOcast is a tall, fixed-angle fracture walking boot used for clinician-directed immobilization of the foot, ankle and lower leg. It combines a rigid four-strap shell, vacuum-bead insert, washable cloth liner and detachable rocker sole.
What injuries may the VACOcast be used for?
VACOcast may be prescribed for ankle fractures, metatarsal fractures, calcaneal fractures, selected foot or lower-leg fractures, severe sprains, complex ligament injuries, arthrodesis and selected post-operative foot or ankle conditions.
Is VACOcast the same as a CAM boot?
VACOcast is a type of medical walking boot or fracture orthosis and may be described as a CAM-style boot. However, unlike boots with adjustable ankle motion, VACOcast holds the ankle at a fixed 90° position.
Does VACOcast replace a plaster or fibreglass cast?
OPED describes VACOcast as a cast-stable orthosis that may be used through different treatment stages. Only the treating physician or surgeon can determine whether it is an appropriate alternative to a traditional cast for a particular injury.
Is VACOcast suitable for an Achilles tendon rupture?
VACOcast holds the ankle at a fixed 90° position and does not include an Achilles wedge sole or adjustable range of motion. Achilles rupture protocols often require plantar-flexion positioning and progressive angle changes, making the VACOped XELGO the more specialized OPED option. Follow the treating clinician’s prescription.
What is the difference between VACOcast and VACOped XELGO?
VACOcast is a fixed 90° fracture boot with a vacuum-bead insert and hand pump. VACOped XELGO can be configured in a fixed position or through a controlled range of motion and uses a pump-free adaptive liner. VACOped XELGO also includes an Achilles wedge sole.
How does the VACOcast vacuum insert work?
The insert contains thousands of small beads. When air is removed with the supplied pump, the beads settle around the foot, ankle and lower leg and become firm. This creates an individualized supportive contour without relying on inflatable air chambers.
Is VACOcast an air walking boot?
No. It does not use inflatable air cells. VACOcast removes air from a bead-filled cushion so the insert becomes firm around the individual shape of the limb.
Can the VACOcast be readjusted when swelling changes?
Yes. The vacuum can be released so the beads become flexible and can be redistributed before the insert is remoulded. Significant changes in swelling, pain or fit should be discussed with the treating healthcare professional.
Is the VACOcast boot adjustable at the ankle?
No. VACOcast holds the ankle at a fixed 90° position. It does not have the adjustable range-of-motion mechanism found on the VACOped XELGO.
Can VACOcast be worn on either foot?
Yes. VACOcast has a universal design and can be worn on the left or right foot.
What sizes are available?
VACOcast is available in Small, Medium and Large. Small fits approximately men’s US 3.5–7.5 and women’s 4.5–8.5. Medium fits men’s 7.5–11.5 and women’s 8.5–12.5. Large fits men’s 11.5–15 and women’s 12.5–16.
Which VACOcast size should I choose if I am between sizes?
OPED generally recommends considering the smaller size when a shoe size falls directly on a size boundary. Foot width, ankle circumference and calf circumference must also be considered, so contact The Physio Store before ordering if your measurements are close to a boundary.
Will VACOcast fit a large calf?
The Large fits a calf circumference of approximately 15–23 inches, while Medium fits approximately 13.5–20 inches. OPED offers additional liner extensions and longer straps for some users. Contact The Physio Store to confirm compatibility before ordering these accessories.
What comes with the OPED VACOcast?
Each VACOcast includes the rigid orthosis, one vacuum-relief insert, one hand pump, one removable cloth liner and one detachable rocker walking sole.
Does the VACOcast include a second liner?
No. The current standard kit includes one cloth liner. A same-size replacement liner can be purchased separately and is useful while the original liner is being washed or air-dried.
Can I wash the VACOcast liner?
Yes. Remove the vacuum insert before washing the cloth liner. The liner may be hand washed or machine washed in cold water using mild detergent. Air-dry it completely and do not use a tumble dryer.
How do I clean the plastic shell and rocker sole?
Wipe the shell and sole with a damp cloth and mild soapy water. Dry all components completely before reassembling and wearing the boot.
Can I swim while wearing VACOcast?
OPED states that the shell and vacuum components can be used in water, but swimming requires prior approval from the treating surgeon or healthcare professional. Do not expose an open wound or incision to water. Bring a dry replacement liner and change into it immediately after leaving the water.
Can I shower while wearing VACOcast?
Approval to use VACOcast in a pool does not automatically make standing in a shower safe. Wet surfaces increase the risk of falling, and wounds may need to stay dry. Follow the treating professional’s instructions and use an appropriate shower cover or seated bathing method when recommended.
Can I walk immediately in the VACOcast?
Only if weight bearing has been approved by the treating healthcare professional. Depending on the injury and healing stage, the patient may be non-weight-bearing, partially weight-bearing or fully weight-bearing.
Do I need crutches with the VACOcast?
That depends on the prescribed weight-bearing status, balance and walking ability. A physician, physiotherapist or other treating professional should determine whether crutches or another mobility aid is required.
Can I sleep in the VACOcast?
Wear during sleep depends on the injury, surgery and treatment protocol. Do not remove the boot at night unless the treating healthcare professional has said it is safe to do so.
Can I drive while wearing VACOcast?
Do not drive unless the treating healthcare professional has confirmed that it is safe and you can operate the vehicle normally. Driving with a right-sided immobilization boot is particularly concerning and may affect insurance coverage or legal responsibility.
How long should I wear the VACOcast?
The wearing period depends on the diagnosis, healing progress and treatment plan. Only the treating physician, surgeon or rehabilitation professional can determine when the boot may be reduced or discontinued.
Is VACOcast designed for running or sports?
No. VACOcast is an immobilization and protected-walking orthosis. It is not designed for running, jumping, fast walking or sporting stresses.
Why might I need an EVENup shoe lift?
A tall walking boot raises the injured side and may create a temporary leg-length difference. An EVENup worn over the regular shoe on the opposite foot may help improve symmetry and reduce strain on the hips, knees or lower back when approved by the treating professional.
Can replacement VACOcast parts be purchased separately?
Yes. Replacement cloth liners, vacuum inserts and selected straps or accessories are available. Match replacement parts to the size and model of the existing boot and contact The Physio Store if compatibility is uncertain.
Is the regular VACOcast suitable for diabetic foot ulcers?
The standard VACOcast fracture boot should not be substituted for the specialized VACOcast Diabetic model. Diabetic wounds and reduced sensation require professional assessment, pressure management and frequent skin monitoring.
What should I do if the boot causes numbness or a pressure area?
Stop weight bearing and contact the treating healthcare professional if you experience numbness, altered sensation, discolouration, increasing pain, skin breakdown, unusual swelling or a persistent pressure point.
Do I need a prescription to purchase VACOcast?
A prescription is not normally required simply to purchase the boot. However, professional assessment is strongly recommended, and a valid prescription may be required for insurance reimbursement or applicable tax-exempt processing.
Is the VACOcast tax-exempt in Canada?
An orthopaedic device may qualify for tax-exempt processing when the applicable Canadian requirements are met, which may include a valid prescription. Upload the prescription using the product-page form so The Physio Store can review the documentation before processing the order.
